Home Health Physical Therapist Assistant (PTA) | Porter County, IN
One Home Health is seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our growing home health team serving Porter County, Indiana, including Valparaiso, Portage, Chesterton, and surrounding communities.
Pay: $45-$55 per hour Schedule: Full-Time, Part-Time, or PRN
What You'll Do
Provide physical therapy treatments under the supervision of a licensed Physical Therapist.
Help patients improve mobility, strength, balance, and independence in their homes.
Educate patients and caregivers on exercises, safety, and fall prevention.
Complete documentation and communicate patient progress with the care team.
Qualifications
- Active Indiana PTA license.
- Current CPR/BLS certification.
- Valid driver's license, reliable transportation, and auto insurance.
- Strong communication and documentation skills.
- Ability to travel throughout Porter County.
Preferred
- Home health experience.
- Knowledge of Medicare documentation requirements.
- Experience with Axxess software.
