Are you a compassionate, dedicated Registered Nurse who enjoys building meaningful relationships with patients and families? We are looking for an enthusiastic and skilled Home Care/Hospice Registered Nurse to join our growing team serving the Lansing, Michigan area.
This is an opportunity to provide personalized, patient-centered care in the comfort of patients' homes while making a meaningful difference in the lives of individuals and their families.
Position Responsibilities
- Provide skilled nursing assessments and direct patient care in the home setting.
- Develop, implement, and evaluate individualized plans of care.
- Perform comprehensive patient assessments and monitor changes in condition.
- Provide medication management, wound care, disease management, and patient education.
- Coordinate care with physicians, therapists, social workers, aides, and other members of the interdisciplinary team.
- Provide compassionate hospice care, including symptom management and end-of-life support.
- Educate patients and caregivers regarding disease processes, medications, safety, and care needs.
- Complete accurate and timely clinical documentation in accordance with agency policies and regulatory requirements.
- Communicate effectively with patients, families, physicians, and the clinical management team.
Qualifications
- Current, unrestricted Michigan Registered Nurse (RN) license.
- Previous home health, hospice, or acute care experience preferred.
- Strong clinical assessment and critical-thinking skills.
- Excellent communication and organizational skills.
- Ability to work independently and manage a flexible daily schedule.
- Valid driver's license, reliable transportation, and automobile insurance.
