We are looking to build our Registered Nursing team across the Metro North Brisbane area and are interested in hearing from experienced nurses who bring strong clinical capability, a commitment to person-centred care and the ability to work confidently in a community setting.
About the Role
As a Registered Nurse, you will provide comprehensive nursing assessments, clinical care and case management to clients in their own homes.
You will work independently in the community while collaborating closely with clients, families, support workers and other health professionals to ensure clients receive safe, high-quality and coordinated care.
Your responsibilities will include:
- Conduct comprehensive nursing assessments and develop appropriate care plans.
- Provide clinical care based on individual client needs.
- Undertake ongoing assessment and monitoring of clients’ health and wellbeing.
- Coordinate and manage client care in collaboration with relevant health professionals.
- Identify and respond appropriately to clinical risks and changes in client condition.
- Provide education and support to clients and their families.
- Maintain accurate and timely clinical documentation.
- Work within relevant professional standards, legislation, policies and procedures.
- Apply infection prevention and control principles and safe clinical practices.
- Build positive and respectful relationships with clients and their support networks.
- Work collaboratively within a multidisciplinary team.
- Manage your workload and appointments effectively while working independently in the community.
About You
We are looking for Registered Nurses who demonstrate strong clinical capability and a genuine interest in supporting people to remain living independently in their own homes.
Community nursing experience is highly desirable. We will also consider applicants with clear transferable clinical skills and experience from other areas of nursing who can demonstrate the ability to transition successfully into a community-based role.
You will ideally have:
- Demonstrated clinical experience within a community service, home care, aged care or another relevant clinical setting, or strong transferable skills.
- The ability to independently assess client needs and provide appropriate clinical interventions.
- Strong clinical judgement and an understanding of risk management.
- Excellent time management and organisational skills.
- The ability to work autonomously while knowing when to seek support or escalate concerns.
- Strong communication and interpersonal skills.
- The ability to build positive relationships with clients, families and multidisciplinary teams.
- A person-centred approach to nursing care and a commitment to supporting clients' independence and wellbeing.
Mandatory Requirements
- Current AHPRA registration as a Registered Nurse.
- Degree in Nursing or equivalent qualification and experience to Registered Nurse level.
- Evidence of recent nursing practice.
- Current Australian Driver Licence.
- Access to a reliable vehicle for work-related travel.
- Current CPR certification.
- Either a current National Police Certificate or NDIS Worker Screening Clearance.
