Woolwich Community Health Centre (WCHC) requires a part-time 22.50 hours per week (0.6 FTE) clinical nurse (RN) to provide day and evening coverage for our St. Jacobs, Wellesley, and Linwood sites. The Clinical Nurse functions as a member of the inter-disciplinary team by providing a range of nursing services essential to client care.

Position Summary:

The Registered Nurse (RN) provides comprehensive, evidence based primary care services as part of an interprofessional team. The RN supports health promotion, disease prevention, chronic disease management, episodic care, and care coordination while practicing to the full scope of the Registered Nurses of Ontario (CNO) standards.

In accordance with Ontario legislation and regulatory requirements, qualified RNs with approved prescribing education and authorization may prescribe medications within their legislated scope, contributing to improved access to timely, patient-centered care.

The RN works collaboratively with physicians, nurse practitioners, allied health professionals, and community partners to deliver high-quality, culturally safe, equitable, and accessible care.

Requirements:

Required

• Current registration as a Registered Nurse in good standing with the College of Nurses of Ontario.
• Minimum 2–3 years of clinical nursing experience, preferably in primary care, community health, family practice, or chronic disease management.
• Demonstrated assessment, clinical reasoning, and patient education skills.
• Experience working within an interdisciplinary team environment.
• Proficiency with electronic medical records.

Preferred

• Primary Care Nursing Certificate or equivalent training.
• Experience in community health centre settings.
• Chronic disease management certification.
• Immunization certification.
• RN Prescribing education and authorization in accordance with Ontario legislation and CNO requirements.

Scope of Practice Statement

• The RN practices to the full extent of their legislated scope and competencies. Qualified RNs with prescribing authority will exercise that authority in accordance with provincial legislation, College of Nurses of Ontario standards, organizational policies, and established clinical governance processes. Prescribing activities will be supported through ongoing competency development, quality assurance processes, and collaborative care practices.

Hours and location of work would be pre-arranged, based on coverage needs at any of the facilities.
