Job Description

Job Title: CNA/HHA
Division / Dept: Home Health, Hospice, Private Care
Reports To: Patient Care Manager or designee

Job Summary: Perform and and/or assist with activities of daily living (ADL) and personal care as written in the client’s individualized plan of care. Work under the supervision of the Registered Nurse (RN) or Therapist. Receive scheduling assignments and scheduling direction from the PCM or Scheduler. Integrate and demonstrate the core values of the VNA in all aspects of work responsibilities.

Essential Job Duties/Responsibilities:
The individual must be able to perform the essential job duties and responsibilities to be effective. Other duties may be assigned as needed

1. Perform ADL and personal care activities for the client according to the written assignment by the RN. Per State regulations the CNA/HHA may:
◦ Apply external catheters
◦ Give or assist with personal care, e.g., care of mouth, skin, hair or other care as assigned by the professional nurse
◦ Assist to bathroom, commode or in using bedpan or urinal
◦ Assist with transfers and ambulation with or without an assistive device
◦ Assist with prescribed Range of Motion (ROM) and prescribed exercises which the client and HHA have been taught by the appropriate professional personnel or written instructions left in the home
◦ Assist the client with eating, meal preparation including special diets prescribed for the client
◦ Assist the client with relearning household skills
◦ Take temperature, pulse, respiration and blood pressure-
◦ Perform Foley catheter care
◦ Reinforce dressings, if needed, and contact RN Nurse Case Manager, Patient Care Manager or Director
◦ Document personal care given on client's daily record
◦ Assist client with self-administered medications with completion of AHCA approved course for Assistance in Self Administration of Medications
◦ Assist with emptying colostomy bag if taught and supervised by RN

2. Provide safe, competent care to promote client health, hygiene, and comfort
3. Use/store equipment and supplies in a safe, appropriate manner; Follow VNA safety policies/procedures. Report accidents or unusual incidents immediately to the supervisor and People & Culture
4. Provide light housekeeping tasks such as meal preparation, dishwashing, laundering client’s garments, linen change, and dusting and vacuuming client’s room. Provide pet care if needed
5. Private duty aides: Provide transportation for the client to physician appointments, errands, outside activities etc., as required
6. May assist with installation and troubleshooting of telehealth devices in the patients’ homes
7. Follow HIPAA guidelines and maintain all protected health information (PHI) obtained in the course of work as confidential and, as such, should not be a matter of discussion with co-workers or others outside of the Agency
8. Assist the client to reach maximum potential with mobility and activities of daily living under the direction of the RN or Therapist
9. Document client’s condition in EMR or telephony reports to ensure continuity and quality of care and maintain clinical records according to agency guidelines
10. Observe and report client physical and mental changes to RN Nurse Case Manager, Patient Care Manager or Director
11. Report accidents or unusual incidents immediately to the Director, Patient Care Manager or RN Nurse Case Manager
12. Comply with VNA policies and procedures
13. Follow safety practices, procedures and policies. Report client safety hazards to supervisor. Report work-related injury/illness immediately to Director and People & Culture
14. Attend required meetings and complete mandatory education by the department deadline

Special Conditions of Employment:
1. May not cut the patient’s nails
2. May not perform the following per State regulations:
• Apply heat under any conditions
• Apply cold without physician orders
• Perform gastric gavage or lavage
• Disimpact or give suppositories
• Administer enemas
• Insert urinary catheters
• Administer medications
• Insert nasogastric or gastrostomy tubes
• Irrigate a colostomy or wound
• Pack decubiti or surgical wounds
• Insert or remove I.V. lines
• Care for a tracheotomy tube, nor any other personal health service, which has not been included by the registered nurse in the patient care plan
• Change sterile dressings
• Change mechanical ventilator setting
• Change prescribed oxygen settings
• Perform vaginal irrigations
• Remove sutures or surgical drains
• Perform any procedure without prior instruction
• Perform any procedure clearly defined as a nursing function by the Florida Board of Nursing

Qualifications:
To perform this job successfully, an individual must have the following.

• Graduate of CNA Program with a minimum of 75 hours (HHA) including 20 hours of Home Health Education in the course curriculum or Graduate of 40 hour Home Health Aide program, and/or the VNA reserves the right to test Home Health Aide competencies
• Current CPR certification
• Minimum six months work experience in home health, hospice or acute/long term facility
• Current and valid Florida driver’s license
• Current automobile insurance per VNA requirement

Physical Requirements:
To perform the essential duties, the individual must be able to:

Regularly: sit, stand, walk, balance, talk, hear, feel, and touch; reach and grasp with hands/arms; use hand-eye coordination, use vision for close, peripheral, depth, and ability to adjust focus
Occasionally: stoop, bend, kneel, crouch, crawl and climb; carry/lift up to 50 pounds and/or maneuver over 350 pounds with or without devices.

Work Environment: Time is spent in the client home environment with moderate noise level. Frequent local travel is necessary within Indian River or adjacent county. Client home environment may expose associate to temperature or weather extreme; physical injury by client, family member, animal or other in the community; unsafe environment or exposure to infectious disease.
