Medical Social Work Care Manager &… at Unicity Care… · Paramus

Why social workers take this role

This is care management with real autonomy. You will carry a small, intimate private-pay caseload — deliberately kept that way — so you actually know your clients: their history, their family dynamics, what a good day looks like for them. You will build your own schedule around your visits and exercise real professional judgment, without a discharge queue, a productivity dashboard, or a supervisor assigning your day.

• $70,000–$90,000 base, plus a monthly performance bonus earned on billable hours and paid every month, never deferred to year-end
• No hospital pace. Weekdays, hybrid, with genuine flexibility in how you build your week
• Dedicated company phone number — clients reach you on a company line, not your personal number; laptop provided
• Mileage and tolls reimbursed
• Health, dental, and vision coverage sponsored by the company
• Safe harbor 401(k) — company contributions are fully vested from day one, with no waiting period to earn them
• 15+ PTO days plus paid holidays
• Voluntary life insurance, short-term disability, and AD&D coverage

About the role

Unicity Care Management supports individuals and families through the healthcare decisions that matter most — a new diagnosis, a complicated hospital discharge, a chronic condition that has outgrown what one family can coordinate on its own. Our clients range widely in age and circumstance. What they share is a moment where the healthcare system has become more than they can navigate alone.

As a Social Work Care Manager and Patient Advocate, you are the person your clients and their families call first.

You will:

• Conduct comprehensive in-home assessments covering functional, cognitive, psychosocial, and environmental needs
• Develop and maintain individualized care plans, revising them as circumstances change
• Coordinate physicians, home care, specialists, and community resources into a single coherent plan
• Support clients and families through transitions of care, closing the gaps where people are most often lost
• Provide emotional support and crisis intervention at the moments families need it most
• Advocate for clients at appointments, care conferences, and points of decision
• Serve as the primary point of contact for the clients on your caseload, including responding to urgent needs that arise outside standard business hours
• This is a field-based role requiring regular travel and is not an in-office position. Provides high-touch, concierge-level nursing support to clients, including accompanying clients to medical appointments and other care-related visits as needed.

What we are looking for

• Active Licensed Social Worker (LSW or LCSW) in New Jersey
• A genuine interest in working alongside families, not only clients
• Sound professional judgment in community settings, where the resources of an institution are not at hand
• Strong written and verbal communication; you will be the clearest voice in the room for families under stress
• Organizational discipline to manage an independent caseload and maintain accurate documentation
• A collaborative approach to working with physicians, providers, and community partners
• Valid driver's license and reliable transportation for field visits

Our commitment

Unicity Care Management serves families at moments when the healthcare system is at its least navigable. We hold ourselves to disciplined operational standards so our care managers spend their time on clients rather than on administrative friction. If you want to practice social work as it was meant to be practiced — with time, relationship, and authority to act — this role was built for you.
