Role Overview
The RN Nurse Navigator is a specialized clinical resource nurse dedicated to Trusted Ally Home Care's Skilled Service Line (SSL). The primary purpose of this role is to maximize clinically appropriate utilization of authorized SSL opportunity hours through direct patient visits, proactive opportunity identification, nurse education, and coordination with Clinical and Scheduling teams. This position does not maintain a traditional SCM caseload; its focus is SSL execution, training, and capture of available nursing hours that may otherwise go unutilized.
5 Major Job Functions
1. SSL Visit Delivery & Resource Coverage
Complete SSL nursing visits across assigned patients and markets using available authorized nursing hours. Function as a flexible RN resource to fill clinically appropriate SSL opportunities, support continuity of care, and reduce unused nursing capacity.
2. Opportunity Hour Identification & Utilization
Proactively identify open, available, or underutilized SSL nursing hours and work with Scheduling and Clinical leadership to convert those opportunities into completed visits. Maintain consistent visibility into remaining opportunity hours and barriers to utilization.
3. SSL Training & Nurse Development
Train, coach, and support RNs on the SSL model, visit expectations, clinical workflows, documentation standards, and IGEA requirements. Provide hands-on education and reinforcement to build field nurse competency and confidence in SSL delivery.
4. SSL Documentation, Quality & Compliance
Complete timely, accurate, and compliant documentation for all Nurse Navigator visits and reinforce SSL documentation standards with field nurses. Identify recurring documentation, workflow, or clinical quality gaps and escalate trends for education and process improvement.
5. SSL Opportunity Management & Clinical Collaboration
Partner with SCMs, DON/RDON leadership, Scheduling, and Clinical Operations to remove barriers preventing authorized SSL hours from being delivered. Track and communicate opportunity hours identified, hours scheduled, visits completed, hours captured, and remaining opportunities.
Key Responsibilities and Duties
- Complete SSL visits in accordance with the patient plan of care, authorization, physician orders, TAHC policy, and applicable regulatory requirements.
- Review assigned SSL opportunity data and prioritize available hours that can be safely and appropriately utilized.
- Coordinate with Scheduling to convert identified opportunity hours into patient visits as quickly and consistently as possible.
- Engage patients and families to support understanding, acceptance, and continuity of SSL nursing services when additional authorized hours are available and clinically appropriate.
- Provide SSL orientation, ride-along support, competency reinforcement, and ongoing coaching for field RNs.
- Ensure IGEA and all required SSL documentation is completed accurately and within established timelines.
- Identify utilization barriers including patient availability, staffing, scheduling, training, documentation, or workflow issues and escalate them to the appropriate owner.
- Maintain clear communication with the primary SCM and clinical leadership without assuming ownership of the patient's ongoing case-management responsibilities.
- Support expansion of SSL capacity by helping additional RNs become competent and comfortable performing SSL visits.
- Participate in SSL performance reviews and provide actionable information regarding utilization opportunities, barriers, and field education needs.
- Maintain patient confidentiality and HIPAA compliance at all times.
Education and Experience Qualifications
- Graduate of an accredited school of professional nursing.
- Active Registered Nurse (RN) license in the state(s) of practice; compact/multistate licensure preferred where applicable.
- Minimum of two years of professional nursing experience; home health, community-based care, case management, or complex chronic care experience strongly preferred.
- Experience educating or precepting nurses and supporting clinical competency development preferred.
Required Qualifications
- Home health or in-home nursing experience.
- Experience with DOL/EEOICPA populations or other authorization-driven home care programs.
- Previous experience with SSL workflows, IGEA documentation, or TAHC clinical processes.
- Experience training, precepting, or mentoring field nurses.
- Experience working across multiple patients, locations, or markets as a resource/float nurse.
Preferred Qualifications
- Utilization Mindset - Consistently identifies clinically appropriate opportunities to maximize authorized SSL hours without compromising patient-centered care.
- Clinical Excellence - Delivers safe, skilled, high-quality nursing care and recognizes changes requiring escalation.
- Teaching & Coaching - Builds nurse competency through practical education, demonstration, feedback, and reinforcement.
- Ownership & Follow-Through - Moves identified opportunities from discovery to scheduling to completed visit and communicates barriers early.
- Collaboration - Works effectively across Clinical, Scheduling, Operations, patients, families, and field staff.
- Documentation Discipline - Maintains accurate, timely, compliant clinical records and reinforces the same standard with others.
- Adaptability - Functions effectively as a flexible resource across changing patient needs, schedules, and service areas.

- SSL opportunity hours identified, scheduled, and completed.
- Timeliness and compliance of SSL/IGEA documentation.
- Number of RNs trained, supported, and validated for SSL delivery.
