About Tokaido Health

Pharmacy is the fastest-growing line item in American healthcare, and most of that growth has nothing to do with new science. It's misaligned incentives and members who can't reasonably navigate the system on their own.

The Role

You'll work directly alongside our founding team, shipping production systems at the intersection of healthcare data, AI, and patient-facing tooling. Real ownership across the stack, genuine mentorship from world-class engineers who've built at scale, and the autonomy to drive meaningful parts of the product yourself.

What You’ll Do

This is the surface area we're building across. You won't touch all of it on day one, but it's the world you'll be living in.

• Agentic clinical workflows. Multi-step agents that reason across claims, formularies, geography, and member preference to surface the right intervention at the right moment. We're designing for agents as first-class participants in the system, not bolt-ons.
• Multimodal outreach and engagement. Voice, SMS, email, and chat agents that proactively engage both clinicians and patients to move a decision forward. Real human navigators stay in the loop where it matters.
• LLM-centric knowledge management. A clinical and payer knowledge layer structured so LLMs can reason over it reliably. Retrieval, evaluation, and grounding are core engineering disciplines here, not afterthoughts.
• Member-facing applications. The surface where behavioral economics meets software. Incentive flows, switch journeys, reimbursement, and working with patients and clinicians to incentivize responsible healthcare actions.

What We’re Looking For

We care about aptitude, curiosity, and builder instinct. Not years of experience or a specific tech stack.

• Strong software engineering fundamentals. You pick up new languages and domains quickly because you understand the underlying principles.
• Genuine enthusiasm for AI-driven development. You're already using LLM-based tools and you're excited about where this is heading.
• A bias toward shipping. You'd rather build a working prototype than debate architecture for two weeks.
• Comfort with ambiguity and intellectual curiosity about healthcare. You're energized by the complexity and the stakes.
