We are seeking a confident and organised Service Coordinator to provide operational coverage during early mornings, evenings and weekends, key service periods where strong coordination ensures continuity of care for both participants and frontline staff.
This role is ideal for someone who enjoys real-time coordination, problem solving and supporting teams in a fast-paced environment.
You will work independently within established policies and escalation pathways, with support from the broader coordination and management team. The role will sit under Senior Service Coordinator for day-to-day task direction and support.
Office Hours:
Monday to Friday: 09:00am to 05:00pm
There is an after hours on-call component also part of the role, Roster for on-call is rotational between all Service Coordinators covering 5pm to 9am on weekdays and full days on Saturdays and Sundays. Roster will be set and communicated well in advance by the reporting manager.
On-call allowance will be provided for rostered days.
Penalty rates apply in accordance with the SCHADS Award - Social and Community Level 3.3 - $40.62 per hour
About the Role
As a Service Coordinator you will support continuity of services during critical service periods.
Responsibilities include:
- Coordinating roster changes and managing short-notice call-ins
- Arranging shift coverage within approved compatibility guidelines
- Responding to low to moderate incidents and service concerns
- Ensuring incidents are documented and escalated appropriately
- Maintaining accurate participant and service documentation
- Assisting with creating participant support plans, risk assessments and service agreements
- Communicating and collaborating with support workers and participants on daily basis

Essential Requirements
- Certificate III in Individual Support or equivalent qualification
- Minimum 2 years experience in the disability, aged care, community services or a similar high-demand service environment
- NDIS Worker Screening Check
- Current First Aid & CPR
- Current Driver’s Licence and access to a vehicle
