Job Description
Summary: Floyd County School District is seeking a Community Health Liaison to facilitate and support the implementation of healthcare partnerships within the school division. This role involves coordinating communication between schools, families, and healthcare providers to enhance student access to care and promote overall well-being.
Responsibilities:
- Serve as the primary on-the-ground point of contact between school staff, families, and Tri-Area Community Health Center personnel
- Facilitate clear, aligned communication between clinical partners and school administration regarding scheduling, program logistics, and student needs
- Manage the day-to-day logistics of the health program, including scheduling appointments and coordinating student availability with teachers
- Oversee the physical or virtual coordination of care; ensure the telehealth space and equipment are properly prepared, secure, functional, and confidential for student sessions
- Distribute, track, and collect necessary parental consent forms and intake paperwork
- Actively engage with families to introduce available health services, answer logistical questions, and assist them in navigating the enrollment process
- Help identify and mitigate barriers keeping families from utilizing services (e.g., lack of technology, language barriers, or difficulty completing paperwork)
- Assist in collecting and organizing non-identifying data and tracking participation metrics required for grant compliance and reporting
- Maintain accurate, up-to-date, and strictly confidential records of program utilization and outreach efforts
- Work collaboratively with school counselors, nurses, and social workers to identify students who may benefit from Tri-Area services
- Help build awareness of the program within the school community through school newsletters, flyers, or brief staff updates
- Maintain the highest level of ethical behavior, strictly adhering to both educational (FERPA) and healthcare (HIPAA) privacy and confidentiality standards
- Perform other relevant health-partnership duties as assigned by the supervisor
- Attends staff meetings, division professional learning workshops, and specialized training sessions as required
- Discharges all responsibilities and fulfills all obligations as specified in the contract, policies, and guidelines of the Floyd County School Board
Required Qualifications:
- High School Diploma or GED equivalency required; an Associate's or Bachelor's degree in human services, health, education, or a related field is preferred
- Strong interpersonal and communication skills, with a demonstrated ability to build trusting relationships with diverse families and medical professionals
- Excellent organizational and time-management skills, with the ability to manage schedules and paperwork efficiently
- Comfortable utilizing basic technology, including virtual meeting platforms (e.g., Zoom, Microsoft Teams) and data entry programs
- Ability to work independently and maintain extreme confidentiality regarding student and medical records
Preferred Qualifications:
- An Associate's or Bachelor's degree in human services, health, education, or a related field
