Terrace Home Health and Hospice is seeking a full time NP/PA to see patients in the Boise, ID area.

Position Description:

The Nurse Practitioner will be responsible for providing care and services for home health and hospice care patients. Visit patients wherever they reside which may include private residences, assisted living facilities, hospitals, long term care as well as group home environments, is available for face-to-face visits working independently yet in tandem with our home heatl/Hospice Team.

Responsibilities Include:

Performing face-to-face encounters with the patients.

Collecting and assessing patient care data, developing and implementing a plan of care, managing and evaluating the outcome of care.

Providing patients and/or families with health education, counseling, and referrals to other healthcare professionals and community resources when appropriate.

Participating in case management.

Ordering treatments and durable medical equipment as indicated.

Who is an ideal candidate? Someone who is:

· EMR proficient.

· Highly organized

· Detail-oriented

· Great communicator (verbal and written)

· Enjoys geriatric patients

· Works well independently

· Timely with documentation

Requirements:

Idaho Nurse Practitioner/Practitioner Assistant license in good standing

Prescriptive authority

Reliable Transportation

Previous home health/hospice experience is preferred but not required
