Help residents and families navigate life's transitions with compassion and expertise.

At RiverMead, community and inclusivity are at the heart of our work. • Identify residents' emotional, social, and environmental needs, and put support plans in place
• Help residents adjust to aging, community living, chronic illness, and changes in health, and find purpose in their new home
• Offer education, support, and resources to residents, families, and staff on dementia, end of life and loss, anxiety, and depression
• Share information on resident rights, living wills, power of attorney, and advance directives on request
