They are seeking a Hospice Clinical Coordinator Assistant to manage medical records, coordinate orders, and assist with incoming calls in their Fort Wayne office. Responsibilities: - Manage Hospice Medical Records blast for your team and follow up on all requests in a timely manner including providing feedback to team member when requests are completed - Complete all routine plotting duties and incoming plotting requests as needed and in a timely manner - Manage all DME orders, pick-ups and follow-up requests as needed in a timely manner - Call funeral homes daily on any previous day patient deaths and follow death certificate physician verification process - Order x-rays & labs as requested - Answer large amounts of incoming calls from patients, families and referral sources and show professionalism and competency in assisting and/or directing calls to the appropriate department Required Qualifications: - Experience in medical records and working in an office setting - Experience in customer service - Current driver's license - Must have reliable transportation and insurance Preferred Qualifications: - One year of Hospice experience - CNA License - Preferred Licensed Certified Nursing Assistant in State of Indiana Required Skills: Hospice experience, Certified Nursing Assistant (CNA) license, Medical records management, Competency
