Old Glory Home Health Care & Assistive Care is a locally owned, rapidly growing home health agency serving Richmond and the surrounding communities. We are seeking compassionate, dependable, and motivated Occupational Therapists (OTs) and Certified Occupational Therapy Assistants (COTAs) to join our therapy team.
This is an excellent opportunity for clinicians who enjoy working independently, building meaningful relationships with patients, and helping individuals regain independence and safely remain in their homes.
Responsibilities
Depending on licensure and role, responsibilities may include:
- Evaluate patients and develop individualized occupational therapy plans of care.
- Provide skilled occupational therapy services in the patient's home.
- Help patients improve independence with activities of daily living, mobility, safety, and functional performance.
- Provide therapeutic exercises, functional activities, strengthening, balance, coordination, and adaptive equipment training.
- Assess and address home safety concerns and recommend appropriate modifications or adaptive equipment.
- Educate patients, caregivers, and family members regarding home exercise programs, safety, fall prevention, and strategies to maximize independence.
- Monitor patient progress and modify interventions as clinically appropriate.
- For COTAs, implement established plans of care under the direction and supervision of a licensed Occupational Therapist.
- Communicate patient progress, concerns, and changes in condition with the supervising OT and interdisciplinary care team.
- Coordinate care with physicians, nurses, physical therapists, speech therapists, social workers, home health aides, and other members of the care team.
- Complete accurate, thorough, and timely clinical documentation.
- Participate in care coordination and discharge planning.
- Maintain compliance with applicable home health, Medicare, state, and agency requirements.
Qualifications
Occupational Therapist (OT)
- Current Virginia Occupational Therapist license in good standing.
- Graduate of an accredited Occupational Therapy program.
- Current CPR/BLS certification.
- Strong clinical judgment, communication, and organizational skills.
- Ability to independently manage a home health caseload and schedule.
Certified Occupational Therapy Assistant (COTA)
- Current Virginia Occupational Therapy Assistant license/certification in good standing.
- Graduate of an accredited Occupational Therapy Assistant program.
- Current CPR/BLS certification.
- Ability to provide treatment under the direction of the supervising OT.
- Strong communication, documentation, and patient-care skills.
Preferred for Both Roles
- Previous home health experience is preferred.
- One or more years of clinical experience is preferred.
- Experience working with older adults, medically complex patients, neurological conditions, orthopedic conditions, or patients recovering after hospitalization or surgery is a plus.
- Familiarity with electronic medical records (EMR/EHR).
- Reliable transportation and the ability to travel throughout the assigned service area.

Compensation: $40.00 - $60.00 per hour
