Job Description
Key Responsibilities
- Provide individual counselling using evidence-based approaches tailored to clients' unique needs.
- Conduct psychosocial assessments and develop client-centered service plans, including crisis intervention, referrals, and ongoing support.
- Deliver group workshops and psychoeducational programs.
- Coordinate case management services by connecting clients with internal and community resources and advocating on their behalf.
- Build and maintain collaborative relationships with caregivers, community partners, and external service providers.
- Provide coaching and practical support to clients and families, including financial literacy and budgeting guidance when appropriate.
- Maintain accurate client documentation, case notes, and required reporting in accordance with agency and legislative requirements.
- Participate in interdisciplinary team meetings, program development, and community outreach initiatives.
- Ensure compliance with agency policies, professional standards, privacy legislation (PHIPA), and health and safety requirements.
- Perform other duties as assigned.
What You Will Bring:
- Masters of Social Work or equivalent related degree
- Registered member of OCSWSSW in good standing
- Minimum of 3 years related clinical experience
- Knowledge of Matrix and other data management systems is a strong asset. Knowledge and/or expertise in specific clinical therapeutic interventions, such as Motivational Interviewing, Brief Therapy, CBT, and/or group psychotherapy
- Understanding of and commitment to the Agency’s client centric approach
- Comprehension of French or another language will be considered an asset
