Job Description
The Hospice Admissions Nurse (Start of Care SOC Visit RN) will initiate and develop a plan of care in collaboration with the Interdisciplinary Group (IDG), the patient/family and the attending physician. Provide individualized, in-home coordinated hospice care to bring safety and comfort to the patients and families.
Hours: Full-time | Days | Will be a part of an On-Call and Holiday rotation

Territory: Our office is in Roseville, MN and our service area of where our patients live is in the 11-County Twin Cities Metro area. We are looking for someone who is willing and able to drive anywhere within our service area for patient visits.
A few must-haves for this position:
- Graduate of an accredited school of nursing.
- Licensed to practice as a Registered Nurse (RN) in the state of Minnesota. License is active and in good standing.
- Minimum of twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or, if no experience within the last five (5) years, evidence of current (within the last 12 months) clinical knowledge as deemed appropriate by the Hospice Director.
- One (1) year of hospice experience preferred.
- Able to travel within a geographic area serviced by the office. Reliable transportation, valid driver’s license, and proof of auto insurance are required.
- Proof of current CPR certification.
Some functions of what you would do:
- Completes an initial/comprehensive assessment of the patient/family to determine hospice needs which includes a history of current and previous illness(es), as well as a physical assessment of the patient, with emphasis on the management of pain and other symptoms. Able to complete SOC visits and all required documentation and tasks.
- Assist with bedside consultations for potential referrals/potential patients in private homes, Assisted Living Facilities (ALD), Skilled Nursing Facilities (SNF), or hospitals, including homecare patients with a potential need for/interest in hospice.
- Assist with coverage for PTO, sick calls, LOAs, including acting as RN Case Manager, providing visits, taking calls, etc. as needed.
- Assist with education for patients, families, referral sources, as needed (ex. staff training or presentations for ALF staff).
- Supports business development by assisting the care consultant team with informational sessions, meet and greets, etc. at ALFs, SNFs, clinics, or other referral source and community locations.
