Employment Type: Part-Time, W-2
Hours: Approximately 15–20 hours per week
Pay: $27–$36 per hour, depending on credentials and experience
Location: Chula Vista / South Bay San Diego and surrounding service areas
- Conduct in-home new-client assessments and reassessments.
- Develop, review, and update individualized Plans of Care.
- Conduct scheduled supervisory and quality-assurance visits.
- Follow up with clients and families after new starts of care.
- Monitor client satisfaction and proactively identify concerns that could affect client retention.
- Observe and identify changes in clients' functional abilities, behavior, safety, or care needs and appropriately communicate concerns.
- Help determine caregiver skills, personality, availability, and compatibility for appropriate client-caregiver matching.
- Work closely with the Scheduler/Care Coordinator regarding caregiver assignments and client needs.
- Coach caregivers regarding client-specific expectations, Plans of Care, professionalism, and performance.
- Address client and family concerns professionally and promptly.
- Assist with caregiver performance improvement when concerns are identified.
- Support new starts of care to ensure a smooth transition for clients, families, and caregivers.
- Maintain accurate and timely documentation of assessments, supervisory visits, incidents, client concerns, and follow-up activities.
- Communicate significant concerns to agency leadership and appropriate parties.
- Maintain professional boundaries, confidentiality, and compliance with HomeWell policies.
- Participate in regular care-management and operations meetings.
- Provide a brief weekly report on client-care quality, outstanding concerns, and follow-up needs.
- Current California CNA certification or LVN license preferred.
- Minimum of 2 years of experience working with seniors, preferably in home care, senior living, hospice, home health, skilled nursing, or a related setting.
- Previous experience with client assessments, care coordination, supervisory visits, caregiver supervision, case management, or client services strongly preferred.
- Experience supporting individuals living with dementia, Alzheimer's disease, chronic conditions, mobility limitations, or other age-related needs.
- Strong communication skills and confidence working directly with clients and family members.
- Ability to coach caregivers and professionally address performance concerns.
- Strong assessment, documentation, organization, and problem-solving skills.
- Comfortable working independently in clients' homes and collaborating with an office-based team.
- Comfortable using electronic care-management and documentation systems.
- Valid California driver's license, reliable transportation, and ability to travel throughout the service area.
Client/family follow-ups, weekend issue review, Plans of Care, and caregiver concerns

May occasionally be used for a scheduled assessment or urgent client need
Assessments, quality-assurance visits, caregiver coaching and family follow-ups
Client satisfaction follow-ups, documentation, and preparation for the following week
- Experienced CNA / senior-care professional: approximately $27–$30/hour
- Experienced LVN with strong care-management experience: approximately $31–$36/hour
- Complete required assessments and supervisory visits on time.
- Maintain current and accurate Plans of Care.
- Follow up promptly after new starts of care.
- Respond to client/family concerns within one business day.
- Identify dissatisfied or at-risk clients before concerns escalate.
- Document client and caregiver issues accurately.
- Support strong caregiver-client matches.
- Coach caregivers and follow through on performance concerns.
- Maintain proactive communication with clients and families.
- Help improve overall client satisfaction, quality of care, and retention.
