Social Worker at DaVita in Galion, OH

Posting Date 10/21/2026 525 East Market StreetBldg 50, Akron, Ohio, 44304, United States of America

DaVita is looking for a Dialysis Clinical Social Worker Licensed in Ohio, to work with our dialysis patients based out of our Akron Renal Dialysis in Akron, Ohio.

This position will cover care of patients at three clinics to include:

Akron Renal Dialysis (Akron, OH)

Mid Ohio Dialysis (Ontario, OH)

Galion Dialysis (Galion, OH)

As a Licensed Social Worker at DaVita, you’ll be a part of a Team that values work-life balance and where your personal and professional growth is a top priority.

What you can expect:

• Schedule will be between 32-40 hours per week within - Monday - Friday 8:00am - 4:30pm (some flexibility/variances depending on patient needs)
• Social workers are responsible for the psychosocial health and wellbeing for our patients so we support those needs, providing interventions as needed and complete mandatory assessments, screenings and education
• Our social workers typically have a caseload of around 100-125 patients however that’s spread between the three above mentioned clinics so it’s not that a social work sees that many patients every day
• This position includes three clinics but other than going to your assigned clinics there’s usually a social worker meeting at most 1 per quarter
• Build meaningful and long-term relationships with patients and their families in an intimate outpatient setting.
• Be a part of a Team that appreciates, supports, and relies on each other in a positive environment.
• Performance-based rewards based on stellar individual and team contributions.

DaVita has an open position for a Licensed Social Worker who will be a vital member of each patient’s core care team. For social workers we really look for empathy and good personal skills. Dialysis patients have a lot to adjust to and need a strong social worker to support them. Being able to work independently is very important but there is a lot of support on the social work side. In this vital role, you will help patients understand their rights and responsibilities, and guide them in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease.
