Apply Description Position Summary The Family Home Provider (FHP) is responsible for providing a safe, stable, and supportive home environment for an individual with intellectual or developmental disabilities. The FHP delivers daily care, supervision, and skill-building supports consistent with the individual’s Person-Centered Plan (PCP), while promoting independence, dignity, and community integration. Requirements Essential Duties and Responsibilities Residential Support Provide 24/7 supervision and care in the provider’s home Maintain a clean, safe, and hazard-free living environment Ensure adequate food, clothing, and personal care items are available Support daily living activities (e.g., bathing, grooming, toileting, meal preparation) Health and Medical Support Administer medications as prescribed (per Kentucky MAP guidelines if applicable) Schedule and transport to medical, dental, and therapy appointments Monitor health status and report concerns promptly Maintain documentation of medications, appointments, and health-related incidents Person-Centered Support Implement the individual’s Person-Centered Plan (PCP) and/or Plan of Care Support skill development in: Independent living Socialization Communication Behavioral self-regulation Encourage choice-making and self-advocacy Community Integration Facilitate participation in community activities (church, recreation, employment, etc.) Provide transportation as needed Promote meaningful relationships and inclusion Behavioral Support Implement approved behavior support plans (if applicable) Use positive behavior supports and de-escalation techniques Document and report incidents per agency and state requirements Documentation & Compliance Complete required daily notes and service documentation Maintain compliance with: Kentucky Medicaid waiver regulations (SCL, MPW, or RISE) Agency policies and procedures Adult/child abuse and neglect reporting laws Participate in audits, home inspections, and monitoring visits Collaboration Communicate regularly with: Case managers/support brokers Guardians/family members Agency staff Attend team meetings and required trainings
