An exciting opportunity has become available as the Clinical Care Coordinator in the front line management team at Celebrate May Shaw, Swansea.

Celebrate May Shaw in Swansea incorporates a 50 bed Residential Aged Care Facility, 18 independent living units, an Urgent Care Centre, 3 sub-acute beds and a variety of Support at Home Care packages and services.

The Clinical Care Coordinator is responsible for the day-to-day organisation and functioning of the nursing and care teams, ensuring a high standard of clinical care is delivered to the residents, in accordance with the rights based Act and the statement of rights and Code of Conduct and the relevant organisational strategic direction, policies and procedures.

The employee is required to set and mutually agree priorities with their manager and monitor workflow in the areas of responsibility; this position has the authority to exercise a degree of autonomy within the scope of the role as defined in the role’s accountabilities.

The Clinical Care Coordinator is expected to operate with a degree of autonomy and work in collaboration with the Director of Facility and other management positions as required.
