Job Status: Salaried - Exempt
The Home Care Manager for Buckner Home Care, LLC ensures exceptional care delivery by coordinating resident services, managing caregiver schedules, and fostering strong relationships with residents, family members, and caregivers.
Primary Responsibilities
• Coordinate and maintain resident and caregiver schedules to ensure consistent, high-quality care delivery.
• Create optimal resident and caregiver matches that support strong relationships and positive outcomes.
• Monitor daily scheduling activities and proactively address coverage gaps or changes in resident needs.
• Develop, implement, and maintain contingency staffing plans to ensure uninterrupted service coverage.
• Maintain accurate and up-to-date resident and caregiver information within software system.
• Track, document, and communicate all resident and caregiver interactions, concerns, and service updates.
• Serve as a primary point of contact for residents, family members, and caregivers.
• Respond to incoming calls in a professional, courteous, and knowledgeable manner.
• Follow up promptly on resident and caregiver concerns to ensure timely and satisfactory resolution.
• Build resident loyalty by identifying opportunities to enhance services and improve the overall care experience.
• Recognize and communicate opportunities to increase service hours when doing so would improve care quality and resident outcomes.
• Manage new resident inquiries and gather relevant information to support the resident onboarding process.
• Enter prospective resident information into system and coordinate preparation for care consultations.
• Conduct service inquiries and care consultations to identify resident needs.
• Work with team members to ensure transition from inquiry to service delivery
• Maintain effective communication with leadership, team members, caregivers, residents and family members.
• Participate in caregiver meetings and team discussions as needed.
Other Responsibilities
• Maintain compliance with all Buckner Home Care, LLC policies, procedures, and requirements. Maintain compliance with all federal and state laws. Maintain compliance with all agency regulatory guidelines and requirements.
• Attend staff meetings as required.
• Support and represent Buckner Home Care, LLC, Buckner Retirement Services and its affiliates and subsidiaries at special events, activities and other assigned functions.
• Work successfully as a part of a team and be responsible for developing and maintaining strong working relationships with co-workers.
• Perform general office tasks as required.
• Travel locally to visit client homes on a regular basis.
• Perform special assignments, projects, and other duties as required.
Physical Requirements
• Requires ability to use up to 20 pounds of force occasionally and/or up to 10 pounds of force frequently and/or 5 pounds continuously to move objects.
• Requires sufficient good health to properly discharge duties. Employees shall not be permitted to work for those who have infectious disease or skin lesion for the duration of communicability.\
• Requires ability to drive assigned vehicle(s) or personal vehicle, with appropriate state license, following all laws applicable; must provide proof of liability insurance and must be eligible to be insured under the organizational insurance policy. Must be 21 or older to drive on behalf Buckner Home Care, LLC.
Position Experience & Abilities:
• High school diploma or equivalent required.
• Must meet Texas Home and Community Support Services Agency (HCSSA) requirements to serve as an administrator or alternate administrator.
• Requires one of the following qualifications:
• Must have a high school diploma or a GED with at least one year of experience or training in caring for individuals with functional disabilities; or have completed two years of full-time study at an accredited college or university in a health-related field; or have a Licensed Physician, Registered Nurse (RN), Licensed Social Worker, Licensed Therapist, or Licensed Nursing Home Administrator with a minimum of one (1) year of management or supervisory experience in a healthcare-related setting; or High school diploma or GED with a minimum of two (2) years of management or supervisory experience in a healthcare-related setting.
• Requires the ability to complete and maintain all state-required administrator education, training, and continuing education requirements.
• Must be eligible to serve as an administrator or alternate administrator under all applicable Texas Health and Human Services Commission (HHSC) regulations.
• Requires excellent verbal and written communication skills and the ability to listen effectively.
• Requires knowledge of or ability to learn the senior care industry.
• Requires proven customer service and relationship-building skills.
• Requires strong organizational, prioritization, and time-management skills.
• Requires ability to work independently and handle confidential information with discretion and professionalism.
• Requires effective interpersonal skills and demonstrated problem-solving and decision-making skills.
• Requires strong computer skills and proficiency in Microsoft Word and Excel.
• Requires ability to establish and maintain effective working relationships with residents, families, caregivers, leadership and colleagues.
• Requires ability to work in a fast-paced office environment and manage multiple priorities simultaneously.
• Requires ability to perform duties in a professional office setting.
• Requires availability to work evenings, weekends, and on-call schedules as business needs require.
• Requires ability to sit at a desk and listen effectively on the phone for extended periods of time.
