Overview
Position Overview The Director of Hospital Engagement (Clinical) is a member of our Sales team and works with our referral sources in assisting discharge planners, clients and their families with the transition from facility to home based care. The Director of Hospital Engagement (Clinical) uses his or her knowledge of state homecare regulations to work with the regional Aveanna Healthcare locations to insure a positive outcome for the clients, families, and case managers.Essential Job FunctionsMeet with case managers daily at assigned hospitals.Check on status of new and existing patients, ensure patient and/or caregiver knows what to expect while on service.Create and carry out a specific sales plans and market development plan in with company goals and objectives Provide regular feedback to corporate management regarding sales activity and success in market development Complete documentation for new patient referrals; Work with branch team to ensure we have all appropriate paperwork to set up a patient on service.Respond to patient questions and concerns; assess patient needs, status, and tolerance issues, troubleshoot and make recommendations as indicated or requested.Coordinate efforts with and provide pertinent updates to Payer Relations team throughout patient onboarding processInterface with physicians, dietitians, and case managers to clarify and manage patients’ clinical needsRespond and follow up with branch requests, including insurance issues, medical eligibility, and clinical information for authorizations.Assist with special projects such as specific patient needs in the community, ongoing training requests, hospital discharge processRequirementsBachelor’s degree in Business, Healthcare, or related field; equivalent work experience may be substituted in lieu of degreeMust have a valid driver’s license and driving record that meet company standardsMust be RN, LPN, RT, or RDMaintain valid clinical license or certification including RN, LPN, RT, or RDProficient in Microsoft suite of products including Outlook, Word and ExcelAbility and willingness to travel locallyPreferencesRecent Home Care/Private Duty Experience preferredPrevious experience with Children's Hospital a plusOther Skills/AbilitiesMust maintain company and employee confidentiality at all timesMust maintain professional boundaries at all timesAbility to remain calm and professional in stressful situationsAttention to detailTime ManagementEffective problem-solving and conflict resolutionExcellent organization and communication skillsPhysical RequirementsMust be able to speak, write, read and understand EnglishFrequent lifting, caring, pushing and pulling of up to 25 poundsOccasional lifting, caring, pushing and pulling of up to 50 poundsProlonged walking, standing, bending, kneeling, reaching, twistingMust be able to sit and climb stairsMust have visual and hearing acuityFrequent lifting and repositioning of patientsRepeated or prolonged standing, bending, kneeling, twisting and occasional climbing of stairsAgility and strength sufficient to handle patients and equipment without assistanceMust be able to appropriately respond physically and psychologically to emergency situations in the home or during transportEnvironmentPerforms duties in an office environment during agency operating hoursMust be able to function in a wide variety of environments which may involve exposure to allergens and other various conditionsPossible exposure to blood, bodily fluids and infectious diseasesOther DutiesPlease note this job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the employee for this job.
