Summary: Humana is a healthcare company that specializes in personalized home care through CenterWell Home Health. As a Home Health Physical Therapist, you will help patients regain strength and mobility by delivering personalized therapy in their homes. Responsibilities: - Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes, to attain highest level of physical function - Assess and document patients' physical strengths to support physician evaluation and guide treatment goals - Develop and implement a rehabilitation program consistent with physician's Plan of Treatment and the goals of the patient team - Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation - Document patient observations, interventions, and evaluations promptly and thoroughly - Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries Required Qualifications: - Degree from an accredited Physical Therapy Program (approved by the APTA) - Current and unrestricted Physical Therapy license - Current CPR certification - Experience collaborating with a team of healthcare professionals Preferred Qualifications: - One year of experience as a Physical Therapist Required Skills: Physical Therapy, Physical Therapy license, CPR certification, Collaboration Benefits: Medical, Dental and vision benefits, 401(k) retirement savings plan, Time off (including paid time off, company and personal holidays, volunteer time off, paid parental and caregiver leave), Short-term and long-term disability, Life insurance
