Responsibilities

• Provide direct physical therapy services to residents, including evaluation, diagnosis, and treatment planning.
• Implement and adjust individualized rehabilitation programs to improve mobility, strength, balance, and functional independence.
• Document patient progress, update treatment plans, and communicate outcomes to the interdisciplinary team.
• Reassess residents regularly, modify goals as needed, and record measurable outcomes.
• Collaborate with therapy colleagues, nursing staff, physicians, and case management to coordinate care and discharge planning.
• Adhere to professional standards, safety protocols, and regulatory requirements in a skilled nursing environment.

Requirements

• Active Physical Therapist license in the United States (state-specific requirements apply).
• Ability to work in a skilled nursing facility setting and manage geriatric rehabilitation needs.
• Availability for a 13-week travel assignment with guaranteed hours (35 per week).
• Strong documentation and communication skills; comfortable working within an interdisciplinary team.
• Experience in geriatric or SNF rehabilitation is preferred.
