Responsibilities

• Provide direct nursing care to residents in a long-term care setting according to individualized care plans
• Administer prescribed medications, monitor for adverse effects, and document actions; escalate concerns to the supervising RN
• Conduct routine assessments, monitor vital signs, and participate in care planning; support activities of daily living (ADLs)
• Assist with mobility, transfers, hygiene, nutrition, and memory care needs as appropriate for dementia populations
• Maintain accurate, timely documentation in PointClick Care EMR; communicate changes to the care team
• Float to all units within the building as assigned; respond to patient needs and maintain a safe, clean environment
• Adhere to infection prevention, safety, and quality standards; implement falls prevention and safety protocols

Requirements

• Active New York LPN license
• BLS certification
• 1+ year of LPN experience in a long-term care or skilled nursing setting
• Ability to work in a 40-bed unit with residents including MS, ortho, dementia, and memory care populations
• Proficiency with PointClick Care EMR
• Willingness to work evenings and to float within the facility; every other weekend required
