Job Description
Summary: Gulf Coast Social Services (GCSS) is a dedicated non-profit organization established in 1983, committed to delivering essential human services programs for citizens of Louisiana facing various physical and mental challenges. We are seeking a Support Coordinator to join our team, a role vital in assisting individuals with their needs and ensuring they receive the appropriate resources and support.
Responsibilities:
- Assess client needs and develop individualized support plans
- Coordinate services and resources for clients, ensuring they have access to necessary support
- Act as a liaison between clients, families, and service providers to facilitate communication
- Monitor client progress and adjust support plans as needed to meet changing needs
- Provide guidance and advocacy for clients navigating social services systems
- Maintain accurate documentation of client interactions and service delivery
Required Qualifications:
- Strong interpersonal skills with the ability to build rapport with clients and families
- Excellent organizational skills and attention to detail
- Ability to work independently as well as part of a team
Preferred Qualifications:
- Bachelor's degree in social work, psychology, or a related field preferred
- Familiarity with community resources and social services is a plus
