Job Description
Summary: Ennoble Care is a mobile primary care, palliative care, and hospice service provider seeking a Full-time Hospice On-Call Registered Nurse to provide care and support to terminally ill patients in the Philadelphia area. The role involves collaborating with healthcare professionals, administering medication, and providing emotional support to patients and their families during their final days.
Responsibilities:
- Collaborate with physicians and other professionals to plan care
- Triage phone calls
- Make visits to the patient as needed to evaluate their condition
- Provide support and comfort according to individual patient’s needs
- Work with caregivers to plan and execute excellent patient care
- Help in administering medication
- Alleviate sudden symptoms of the disease’s end phase
- Notify a doctor for a patient’s condition when deemed necessary
- Assist patients and their families with paperwork
- Maintain accurate records
- Help relatives cope with the upcoming loss and provide post-mortem support
Required Qualifications:
- Must be a graduate from an accredited School of Nursing
- Must be licensed in PA as a Registered Nurse by Pennsylvania State Board of Nursing
- One year of RN experience
- Must have excellent assessment skills and problem solving skills
- Should be skillful in organization and in the principles of time management and have knowledge of case management processes
- Must be a licensed driver with an insured automobile in good working order
- The ability to communicate well, both verbally and in writing
- Must have no exceptions to RN license, and be able to administer pharmaceuticals, perform medication reconciliation, and any nursing procedures or special treatments needed within the scope of nursing practice for state
Preferred Qualifications:
- Hospice experience preferred
