Job Description
Summary: Community HealthWorks is dedicated to advancing health equity and improving access to care for underserved communities. The Project Manager is responsible for overseeing the implementation and expansion of CalAIM programming, leading a team of Community Health Workers, and ensuring effective project execution aligned with organizational goals.
Responsibilities:
- Lead, supervise, and support Community Health Workers and/or program staff
- Provide ongoing coaching, feedback, and performance management
- Support staff in navigating complex client needs and day-to-day service delivery challenges
- Foster a team environment that is accountable, responsive, and aligned with program goals
- Develop and manage project plans, timelines, and deliverables
- Monitor progress and adjust as needed to ensure successful implementation
- Ensure program deliverables are completed on time, within scope, and aligned with funder and organizational expectations
- Identify resource needs (staffing, tools, funding) to support program operations
- Ensure program outcomes align with contract, funder, and organizational expectations
- Ensure accurate data collection, documentation, and reporting
- Monitor program performance and support data-informed decision-making
- Prepare reports for internal leadership and external funders
- Build and maintain relationships with community partners, subcontractors, and stakeholders
- Coordinate across internal teams to ensure alignment and effective service delivery
- Maintain project documentation and records
- Escalate risks, barriers, or concerns to leadership as appropriate
- Support continuous improvement of workflows and program operations
- Perform other related duties as assigned
Required Qualifications:
- Bachelor's degree in health, social services, psychology, communications, or a related field; or At least four (4) years of experience in health, nonprofit, or social services; or Equivalent combination of education and experience
- Strong leadership skills with the ability to support, coach, and hold staff accountable in a fast-paced environment
- Ability to manage multiple priorities, adapt to changing needs, and maintain progress toward program goals
- Sound judgment and decision-making skills, particularly in complex or time-sensitive situations
- Clear and effective communication skills with staff, partners, and leadership
- Ability to build and maintain relationships with diverse communities and stakeholders
- Organized and detail-oriented, with the ability to manage documentation, timelines, and reporting requirements
- Commitment to serving underserved populations with cultural humility and respect
- Valid California Driver's License, proof of insurance, and reliable transportation
- Ability to travel within Sacramento County and surrounding areas
- Successful completion of a criminal background check
- Compliance with immunization and safety requirements, including COVID-19 policies as applicable
Preferred Qualifications:
- Supervisory experience preferred
